Cover. In Guayamate, a rural community of sugarcane workers north of La Romana in the Dominican Republic, Marilis and her husband Anol stand in the doorway of their home with their 18-month-old daughter, Yolanda. Both parents are infected with HIV; their first child died at age 2 from AIDS. In her second pregnancy, Marilis received oral AZT during the last month and intravenous AZT during a cesarean delivery. Yolanda received oral AZT beginning at 8 hours of life and continuing for 6 weeks, and formula rather than breast milk. Yolanda is not infected with HIV. Marilis, while not on AZT therapy since delivery, receives Bactrim prophylaxis and dietary supplements, and is clinically improved compared to pre-pregnancy. Both parents approved use of this photograph and the public portrayal of their family's HIV status so as to inspire further efforts to reduce HIV transmission.
